CFM, a well-established (30+ years) physician-owned, highly regarded, collaborative family medicine practice is offering an opportunity to follow your passion. Our single-site facility boasts in-house laboratory testing, EKG, minor procedure rooms for laceration repairs, dermatologic procedures, facture management, a new Paxis 100 digital Straight Arm X-ray machine to expedite our patients’ care as well as an EMR (eCW). Our patients have access to behavioral health providers as part of our practice offerings. Many of our physicians and APPs have developed subspecialties to augment services available to our patients, including migraine and eating disorder treatment, osteomanipulation, infectious disease management, treadmill stress tests, and women’s care. What unique talent and passion might you develop and share with your CFM colleagues and patients? Our eight (8) provider (5 physicians and 3 APPS) group allows our providers scheduling flexibility to accommodate life’s ever-changing demands. We are close to world-class research/resource facilities, OHSU & the College of Osteopathic Medicine of the Pacific-Northwest (COMP-Northwest), where we have a strong history of training the next generation of physicians, and work collaboratively with their medical staff.

This FTE position is a 4 day per week (34 contact hours) schedule with one on-call night approximately every 2 weeks and one weekend approximately every 8 weeks allowing abundant time to enjoy all this wonderful area offers. Our “call” is telephone triage only, as we have coverage for all hospital patients and admissions, and therefore can be done wherever you have phone and internet service. As a private clinic, we have larger latitude for our providers in scheduling, patient panel diversity, and patient volumes. We offer partnership and non-partnership tracks for our physicians.

Job Requirements

  • Board Certified or Board Eligible MD or DO with US residency with current US work visa
  • Board Certified or Board Eligible in Family Medicine
  • Should possess excellent clinical, oral and written communication skills, with a strong commitment to providing excellent team orientated care

Behavioral Health Provider

Position Summary:

Four-day work week, one day remote, excellent benefits, competitive salary, hiring bonus, relocation bonus, and meaningful work. Corvallis Family Medicine, situated in Corvallis, Oregon is seeking a dedicated, highly motivated, integrated behavioral health clinician to join our primary care team. Corvallis is the home of Oregon State University, less than an hour from the Pacific Ocean and from Willamette Valley wine country. We are a close-knit, relational team focused on maximizing work/life balance while providing excellent patient care. Our team strongly values behavioral health and is eager to find the right clinician to contribute their unique expertise and enhance the care we provide.

We serve a patient population that is diverse in age, ethnicity, and socio-economic status. Our practice is committed to improving health outcomes though a medical home model with a strong focus on prevention, chronic disease management, and emotional and mental issues. Our medical providers are passionate about caring for patients with myriad needs. We are a hub for LGBTQ+ care in our community. We provide specialized care for patients struggling with eating disorders, migraines, and chronic pain. Additionally, we have an embedded psychiatrist practicing in the Collaborative Care Management (CoCM) model who will be a consultant and partner in managing behavioral health needs.  We are looking for an independent, flexible, and determined clinician to build a program and have a seat at the table with our medical providers.

Responsibilities:

  • Responsible for providing a full range of mental and behavioral consult services including psychosocial assessment/diagnosis, treatment recommendations, therapeutic interventions, risk assessment, crisis intervention without restrictions in gender, age, or medical comorbidity.
  • Systematically and continually address the patient’s coping mechanisms in collaboration with the patient, care team and family.
  • Utilize appropriate teaching methods and/or referrals to ensure patient/family psychoeducational needs are met.
  • Assist the practitioners by providing behavioral interventions to aid patients in their self-care of medical conditions.
  • Assess, collaborate, and coordinates patient’s needs and readiness with other members of the care team.
  • Work in a fast-paced environment and take appropriate action. Collaborates and builds rapport with patients, staff as well as other members of the team.
  • Document clinical notes in the eCW medical record
  • Provide patient, family and staff education as needed.
  • Maintain knowledge of state specific and federal laws related to mental health, abuse, privacy, duty to warn and other relevant regulations.
  • Engage in constant quality evaluation efforts and ongoing development and improvement of the program.

Requirements for the Position

  • Licensed Clinical Social Worker (LCSW) in the State of Oregon or Licensed Social Worker who is eligible to become an LCSW in Oregon.
  • Working knowledge of: individual assessment, treatment planning and treatment protocols; mental health disorders and diagnostic classifications; Ability to: establish positive working relationships with other employees, individuals and their families, other agencies, and the public; prepare concise and complete clinical documentation related to individual treatment and progress in services in a timely and professional manner; work within a team and collaborate with team members; knowledge of and ability to act professionally with regard to a code of ethics and confidentiality.
  • Knowledge of billing and coding rules for Behavioral Health.
  • Scope of job requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.

 

Preference
Preference will be awarded to candidates who are self-starters with strong relational skills.

 

Salary & Benefits

  • Salary: $80,000-$90,000 commensurate with experience
  • Hiring and Relocation Bonus: $5,000 combined
  • Continued Education: 40 hours + $1,500/yr
  • Four-day work week. Option for one day a week to be remote.

  • Must be Licensed as a Clinical Social Worker in Oregon within the first 30 days of employment,
