An appointment is required to see a doctor at CFM.
If you are already enrolled as a patient at CFM you can schedule an appointment using the Patient Portal or Healow mobile app. This is to schedule appointments more than a week out.
For appointments within a few days please stop by or call us at 541-757-2400.
For CFM patients with urgent medical needs, on-call doctors are available 24 hours a day and same day appointments are available. CFM doctors’ hours vary, so please ask the receptionist about your doctor’s schedule.
Once you have a Patient Portal account you can use Healow to access all your Portal features and information on a mobile application. If you would like access to the Portal, check out the Portal tab on our Patient Center page for details on how to sign up.
The Patient Portal is a website that allows you to:
- Request and keep track of appointments
- View lab results, medications and referrals
- View visit summaries and your personal health record
- Send and receive messages from staff
- Update demographic information
New patients to CFM will need to fill out a new patient application packet. You can initiate this process by calling our office or stopping by. The forms can also be found on this website in the Patient Center section. Complete the forms and bring or fax them to the clinic to request your new provider.
Please note that insurance co-pays are due at the time of your appointment.
