CFM uses an application process for new patients to ensure one of our providers is the best fit for your medical care. To apply to become a patient at CFM, please fill out both pages in the packet below and return them to our office with a copy of your insurance card, both front and back. If there are additional family members who would also like to apply, please fill out a separate packet for each person.
Once we have received your application it will be reviewed by one of our providers who is accepting new patients. You will hear back from us in 7-14 business days. If you need to be seen urgently before this process is completed, you will need to go to an urgent or immediate care clinic.
Preparing for your first appointment
For your convenience, all of the paperwork you will need for the new patient registration process is available below. Please complete, print and sign the required forms and return them to us at or prior to your first appointment. We will request your digital signature on acknowledgements of our privacy practices and financial policy at check in, and offer the opportunity to save a payment method on file or enroll in e-statements.
Arriving at your first appointment
If you haven’t completed the forms ahead of time we ask that you arrive at least 15 minutes early to allow time to fill out the forms. When you come to your appointment you will need to bring your insurance card(s) and a photo ID such as a driver’s license. If you are not the insured person, you will also need to bring their information. We require payment for any co-pays, deductibles, and/or services not covered by insurance at check in.

What is the Patient Portal?
The Patient Portal is your online gateway to accessing medical information and communicating electronically with CFM. You can view summaries of your visits, lab and imaging results, medications and more. The Portal offers a way to securely send and receive messages with CFM. You can also request and view upcoming appointments, all while keeping your information safe.
If you have a smartphone, the Healow app works seamlessly with your Patient Portal account, allowing you to do everything you love about the patient portal in an app interface. It can now also integrate with smart devices to help track your health data and make it accessible to your doctor. This includes fitness bands like Fitbit, smart scales, blood pressure cuffs and blood sugar monitors. If your device is not connectable, you can manually enter information and still get the same benefit.
Interested in a Patient Portal account?
Fill out the consent form and return it to our office with proof of ID. You will be provided with a username and password to access the Patient Portal. The form can be returned to us in person, by postal mail, fax or email upon request.
For any forms sent in, you will need to include a copy of your government-issued ID, and we will call you with your new credentials once the account is created. Consent forms presented at the office will have an account created and credentials printed out for you at that time.
Having trouble logging in?
If you have a cell phone, try the new “Using Mobile Phone” link. Once you’ve logged in you can find your username and update your password in the My Account section.
If you would prefer another method, try the “Trouble logging in” link below the password field. An email will be sent containing your username or a link to reset your password.

No longer interested in the Portal?
We’ll be sorry to see you go, but understand the Patient Portal is not for everyone. If you decide the portal is not a good fit for you, you can cancel your portal account at any time. We will need a note in writing with your full name, date of birth, and a statement that you wish to disable your portal account. You must sign or type your name and today’s date.
